Our Mission StatementThe Mission of Home Health & Beyond Services is to participate as an active part of the community and continuously improve the home health care needs of our patients by delivering value driven compassionate care.

During this period of change, continuity of care is the most important factor in a patient's recovery.Home Health and Beyond provides home health care in the comfort of a patient's home aiding in a faster recovery. Many patients returning from inpatient hospitalization utilizes our services to maintain independent living. Our service provides our patients with the ability to follow their own home rather than checking into a nursing home.Our medical team works closely with our patients and their physicians; designing a care plan tailored to the needs of our patients' lifestyles and medical issues. Our patients' well-being is at the center of every decision we make regarding patient care.Our medical team consists of very compassionate healthcare professionals including registered nurses, physical/occupational therapists, speech pathologists, social workers, counselors, registered dieticians and home health care attendants. All personnel are qualified and meet all state regulations governing their discipline. We service the following counties: Collin, Dallas, Denton, Ellis, Grayson, Tarrant, Kaufman and Rockwall.

About Us


We opened our doors in McKinney, Texas, in August of 2009. Our medical team consists of very compassionate healthcare professionals including registered nurses, physical/occupational therapists, speech pathologists, social workers, counselors, registered dieticians, and home health care attendants.All personnel are qualified and meet all state regulations governing their discipline.We are licensed and certified with the Texas Health and Human Services Commission for Home Health and Personal Assistant Services. We are licensed and certified with Medicare and Medicaid.We service the following counties: Collin, Dallas, Denton, Ellis, Grayson, Tarrant, Kaufman, and Rockwall.


Who is Eligible
• Patient under the care of a physician
• Patient who requires intermittent care
• Patients who are homebound or whose recovery is determined to be better in the home rather than at an institution
Why Home Health Care
• Allows you to receive quality care in the privacy and familiar surroundings of your own home
• Allows family to participate in your care
• Allows you to receive care specifically designed for your condition at a greatly reduced cost
• Decreases the number of days in the hospital
Referrals
• All referrals must be approved by your physician.
• Once a referral is approved and evaluation will be conducted by a registered nurse or a physical therapist

3. Call the office at (972) 369-7308 to set up an interview.
4. For your interview, bring the filled-out and printed application and DPS form as well as your Driver's License and Social Security card.

OFFICE TEAM
Primary functions include, but are not limited to:
Communication including phone, emails, and memos.
Referral intake and benefit verification.
Preparation of admission, orientation, and new hire packets.
Filing, copying, and data entry.
Generating reports, schedules, and regulatory forms.
Organizes, sorts, and maintains documentation in patient files in a timely manner.
Responds to the Agency’s and patient/client needs in a professional and courteous manner to address any concerns and provide support.
Promotes effective communication to patient, families, team members, and other healthcare professionals, while maintaining confidentiality.

HOME HEALTH ATTENDANT
Primary functions include, but are not limited to:
Assist in providing a safe and clean environment/light housekeeping pertaining to patient.
Assist with grooming, elimination, and transfers (from one position to another; for example from bed to chair).
Work cooperatively with the patient, caregiver(s), and family; and share observations and problems with the supervisor.
Light meal preparation for patient.
Linen change/laundry for patient.
Provides an environment which promotes respect for patients and their privacy and property.
Services will be provided at the patient’s residence and as ordered by physician.

LICENSED VOCATIONAL/PRACTICAL NURSE
Primary functions include, but are not limited to:
Administer skilled nursing care under the supervision of a registered nurse (RN) for patients of all ages in their places of residence.
Under the direction of the RN, assists in identifying the patient’s physical, psychological, social, and environmental needs as evidenced by documentation.
Participates in the coordination of care with the interdisciplinary team, patient, family, and referring agency.
Completes appropriate documentation and communication in a timely manner to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes.
Provides services that are ordered by the physician as indicated in the plan of care.
Provides patient and caregiver education.

SKILLED REGISTERED NURSE
Primary functions include, but are not limited to:
Perform start of care assessments, recertifications, hospital transfers, resumption of care, and 60 day summaries.
Observation of post surgical, chronic, acute/unstable conditions.
Perform vitals, blood glucose, weights, oxygen saturations.
Wound care and dressing change.
Nasogastric tube care.
Decubitus care and venipuncture.
Post cataract care.
Bowel/bladder training.
Medication management, administration of injections.
Inhalation therapy.
Disimpaction/enemas.
Teaching of medications, diet, safety, and disease entity.
As ordered by doctor, administration of skilled nursing care for patients of all ages in their place of residence.
Coordination of care with the interdisciplinary team, patients, their families, and the referring agency.
